The Doctor career in The Sims 4 is an active job from the Get to Work expansion pack. Young Adult, Adult, and Elder Sims can join this medical track. You start at level 1 as a Medical Intern, and you can advance to level 10 as Chief of Staff.

Daily schedules and pay growth for doctors
From level 1 to level 4, your Sim works 8 hours per day to reach 40 hours per week. Starting at level 5 as an Assistant Nurse, shifts increase to 10 hours per day. This change gives your Sim more days off while keeping the 40 weekly hours.
Weekly pay starts at 720 Simoleons for a Medical Intern and climbs at each step. A level 7 Doctor - General Practitioner earns 135 Simoleons per hour and 5,400 Simoleons per week. At level 9, a Surgeon takes home 245 Simoleons per hour, or 9,800 Simoleons weekly.
Your Sim also earns paid time off for each work shift completed. A level 1 Medical Intern gets 0.2 days of paid time off per day worked. By level 10, a Chief of Staff earns 0.5 days of paid time off for each shift.
Single track progression and trait pay bonuses
Unlike many jobs in the game, the Doctor career has no branches. Every Sim follows one path up to level 10, where Chief of Staff pays 11,200 Simoleons per week. After reaching the top, each extra overmax level adds 10 Simoleons to your hourly rate.
Specific traits and degrees boost your hourly pay across all 10 levels. A Biology Degree (BS, honors) adds 41%, while a standard Biology Degree (BS) adds 39%. You also get 36% from a Biology Degree (BA, honors) and 32% from a Biology Degree (BA).
High school milestones and other traits increase your Simoleons too. The Knowledge trait grants a 30% bonus, and being Valedictorian adds 15%. Sims who Graduated High School Early or Graduated with Honors receive a 10% pay boost.
Getting promoted faster and using career cheats
When your work performance nears the promotion line, you have a 1% base chance for an early promotion. The Entrepreneurial trait multiplies this chance by 10, and the Growth Mindset moodlet multiplies it by 5. One other condition in the game also affects this rate.
Low performance carries a 100% base chance of demotion at the end of a shift. The Professional Slacker trait changes this multiplier to 0, preventing demotions. If your Sim quits the job, their rejoining level drops by one every 2 days.
