How to become a doctor in BitLife

Keep your Smarts high and finish high school. Study Biology at university. Then go to medical school and graduate. After that, apply for a doctor job in the full-time job list.

1. Childhood and high school. Study hard every year and stay out of trouble.
2. University. Pick a science major. University takes 4 years.
3. Medical school. Apply from the Education menu. Pay with a scholarship, a loan, your parents, or cash.
4. The job. Look for a job with a (Hospital) or (Medical Office) tag. Apply and pass the interview.

We checked these steps in October 2026. We used the BitLife wiki, Destructoid, and Touch, Tap, Play. Some guides are a few years old. Candywriter can change the game in an update, so the details can differ in your version.

New to the game? First read what BitLife is.

You need a university degree in the correct major. You also need high Smarts and good grades. The wiki says that medical school is the hardest school to get into.

Which major should you pick?

Pick Biology. Every source we opened lists it. The sources do not agree on the other majors. Destructoid names Chemistry and Psychology. Touch, Tap, Play names Nursing and Chemistry. The wiki says that the wrong major gets you rejected.

Sometimes the major you want is not in the list. Destructoid says to age up, or close and open the app again. Touch, Tap, Play says to skip a year or go to community college. Then check the list again.

How do you raise Smarts?

Study harder every school year. Go to the library and read books. Destructoid also names documentaries in the Movie Theater and the Memory Test in Mind & Body. For more tricks, read our BitLife hacks guide.

The wiki says that medical school takes 7 years. It also says that it is the most expensive school. A 2022 Touch, Tap, Play guide says 4 years. We trust the wiki number more, but check your own Education screen.

With 4 years of university, you study for about 11 years after high school. That is a long time before your first doctor job.

How do you pay for medical school?

You pick the payment when you enroll. High Smarts give you a better chance of a scholarship. Destructoid warns that a loan can put you in deep debt after you graduate. The wiki says that some countries have free university, but higher education still costs money there.

Can you fail medical school?

Yes. Touch, Tap, Play says that low grades can get you expelled. Check your grades each year and study harder when they drop.

The wiki says that graduate school is an optional step if medical school rejects you. Graduate school takes 2 years. It does not promise a place in medical school.

Finish medical school first. Then look for a surgeon job in the full-time job list. BitLife has two surgeon jobs: Brain Surgeon and Plastic Surgeon.

Brain Surgeon

Destructoid calls the Brain Surgeon one of the highest-paying jobs outside special careers. The job is often near the top of the list. If you do not see it, age up or open the app again. If you fail the interview, try again later.

Destructoid also says that the Brain Surgeon job is part of many weekly challenges. For the full list, read BitLife challenges.

Plastic Surgeon

Candywriter added the Plastic Surgeon job in the Summer 2025 update. Destructoid says that the update added five new careers. The job has a scissor emoji and a (Hospital) tag. It has no fame options, but you can work hard for raises.

Can you be a surgeon with a criminal record?

Probably not. The wiki job table marks the Brain Surgeon as a job you cannot hold with a criminal record. Destructoid tells you to stay away from crime on the way to Plastic Surgeon.

BitLife has no specialty choice inside medical school. Each specialty is a separate job in the job list. A medical degree opens all of them.

The wiki and Destructoid list these jobs that need medical school:

• Family Physician. A (Hospital) job.
• Brain Surgeon. A (Hospital) job.
• Plastic Surgeon. A (Hospital) job from the Summer 2025 update.
• Psychiatrist. A (Medical Office) job. It goes from Jr. Psychiatrist to Psychiatrist to Sr. Psychiatrist.
• Chiropractor. A (Medical Office) job, from Jr. Chiropractor.
• Physical Therapist. A (Medical Office) job, from Jr. Physical Therapist.

Touch, Tap, Play says that you can start in a junior job if the interview fails. You still get the doctor career.

What about nurses, dentists, and vets?

These jobs use other schools, not medical school. The wiki gives these lengths:

• Nursing school: 2 years, for a Nursing major only.
• Dental school: 4 years.
• Pharmacy school: 4 years. The wiki says it is much easier to get into.
• Veterinary school: 4 years.

A new doctor makes about $95,000 a year, by the wiki Careers page. The wiki Careers/Jobs page shows higher numbers. The two pages do not agree, so use the numbers as a rough guide.

We found no salary for the Plastic Surgeon in a source we trust. The wiki says that doctors get a wage that goes up slowly over time. Your pay also depends on the country you live in.

Want more money faster? Compare the doctor path with the actor career or the path to president.

The most common cause is the wrong major. Low Smarts and low grades are the next causes.

• The wrong major. The wiki says that a school rejects you if your major does not match. Art and business majors do not lead to medical school.
• Low Smarts. Raise them before you apply. Go to the library and study harder.
• Low grades. Study harder in your last years at university.
• No way to pay. Without a scholarship, take a loan or ask your parents.
• A hard school. The wiki calls medical school the hardest school to get into. If it rejects you, think about graduate school first.

The wiki also lists a paid shortcut. It says that the Golden Diploma came to the BitLife Marketplace in July 2024. It lets you graduate at once with a postgraduate degree you pick. See is BitLife free for the other purchases.

LifeSim does not fit if you want medical school, a salary, or promotions. LifeSim has no career ladder, no salary, and no medical mini-games. For those, stay with BitLife.

LifeSim fits if you want to live the doctor life, not climb the job list. In the doctor simulator, you describe your doctor and name a real city. You can find a real hospital on the map with place search and go there.

Then you talk to nurses, patients, and friends in your own words. They remember what you said. LifeSim is free and runs in your browser. Read how to play LifeSim to start, or see LifeSim vs BitLife for the full comparison.

Can you become a doctor in BitLife for free?

Yes. Our sources list doctor jobs as normal full-time jobs, not a Job Pack. You only pay in-game money for medical school.

What is the highest-paying doctor job in BitLife?

The Brain Surgeon, by Destructoid and the wiki Careers/Jobs page. The wiki Careers page gives the same starting wage as the Family Physician.

Do you get the title “Dr.” in BitLife?

Yes. The wiki says that your character gets “Dr.” before the name after medical school.

Is there a ribbon or a collection for doctors?

You collect a career when you hold it for 20 years in a row. The wiki says that many schools and high Smarts can give the Academic ribbon. See BitLife ribbons for the full list.

Can a nurse become a doctor in BitLife?

Yes. The wiki lists Nursing as a major that can lead to medical school.
